Account Assignment and Forms

You can download and print all the forms we offer and use your typewriter or computer to complete them. Then send your listings via fax, email or regular mail.

If you wish to list electronically, use the Account Assignment Sheet (for multiple accounts), the General Listing Transmittal (for a single account) or our special transmittal forms for Judgments, Rentals, or Towing Companies.

Simply download the form to your computer. Save a second copy of the form for assigning your accounts— keeping the original as your master for future use. Complete the second form, save your data, close the completed form. E-mail your completed form as an attachment to support@statewidecollection.com.  If needed, use the Fax Transmittal form to fax your additional documents as follow up to your electronic submission of the accounts.  If you wish, you may just complete the form and fax it along with any documents.

Please be as complete as possible. The information requested is an important part of our collection process. Even a disconnected phone number, old employer, or a past mailing address can help locate your lost customer. Please contact us if you have any questions or comments regarding our forms.
